Case Manager

Location: Kew Gardens, NY

Compensation: $35,000 to $40,000

Description:
1)Conduct outreach with hospitals, houses of worship, physicians, disease focused organizations, community-based organizations, and others as identified to enhance value and increase awareness and accessibility of the Caregiver Support Services program.

2)Provide current, appropriate and timely information in response to caregiver inquiries, and provide resource referral options appropriate to caregiver needs.

3)Responsible for all program documentation including caregiver intakes, assessments, and home health care services paperwork. Develop caregiver care plans consistent with caregiver goals. Must also maintain accurate participant files in accordance with NYC DFTA standards.

4)Conduct in-home assessments as needed to determine eligibility for caregiver services

5)Provide one-on-one counseling as indicated by expressed caregiver needs in accordance with NYC DFTA standards.

6)Schedule and provide educational opportunities for caregiver clients.

7)Advocate for clients when necessary.

8)Accurately complete monthly statistics for reporting to lead agency.

9)Make appropriate referrals to Social Adult Day Services program for group respite.

10)Establish case management and appropriate community contacts in participant catchment areas for referral and case coordination. Work with other agency workers and program participants on related social service needs utilizing collateral contacts, I&R, case assistance, case conferences, counseling and advocacy.

11)Develop and lead support groups that accommodate the broad needs of the different populations of caregivers in the community. Expand the meeting time and place of these support groups.

12)Support Social Adult Day program/staff as needed.

13)Attend continuing education seminars as appropriate.  

Qualifications:
Bachelor's degree.
MUST be Bilingual in Russian and English.
